I, the parent or legal guardian of the registered player, acknowledge that participation in activities at 90 Club Soccer Academy involves physical activity and carries a risk of injury. I confirm that my child is physically able to participate and that I will inform the academy of any medical conditions, allergies, or injuries that coaches should be aware of. I understand that in case of an emergency, academy staff may seek appropriate medical care for my child if I cannot be reached immediately. I release 90 Club Soccer Academy, its coaches, staff, and volunteers from liability for injuries that may occur during training or related activities. I also consent to my child participating in all scheduled training sessions and understand that photos or videos may be taken during activities for academy promotional purposes.
